U.S. Passport Services

The Corona Public Library has been a passport acceptance facility to the public since June 1999. Located on the first floor of the library, this service is available Monday through Saturday. Appointments are highly recommended.

Items needed for new or renewal passports:


Please call (951) 736-2386 with questions, to make an appointment, or to verify hours. Due to an increase in passport application volume, acceptance agents may not immediately answer the phone. Please leave a message and a passport acceptance agent will return your call by the next business day.


For New Or Renewal Passport for Adult or Minor age of 16 or over, all applicants must appear in person, and will need the following items:

 
  1. Completed (but not signed) Passport Application Form (DS-11). [Download Form]

  2. Proof Of United States Citizenship Or Nationality -
    These documents MUST BE mailed to the Passport Agency, photocopies are not accepted.

      If you were born in the United States you must submit:

    • Certified copy of birth certificate, or
    • Previous U.S. passport.
    • California Abstract birth certificates or Hospital Birth Certificate are not accepted
     

      If you were born in a foreign country you must submit:

    • Original certificate of naturalization or citizenship, or
    • Previous U.S. passport, or
    • Proof of derivative citizenship.

  3. Proof Of Identity -
    • Current driver’s license, or
    • Recently issued certificate of naturalization, or
    • Recently issued government ID card.

  4. Photographs - Two identical, 2"X2" square, full faced with plain white or off-white background. Passport photos are available at the Library for $10.00.

  5. Passport Fee - 2 checks or money orders only or 1 check/money order and 1 credit card (Visa/MC)

  6. For minors 16 & 17 - at least one parent or guardian must be present to sign and present ID.
    7.  
  7. Photocopies of ID - Photocopies of the front & back of the ID mentioned in Step 3 must be submitted with a passport application.  The copies must be single-sided and legible.  Copies can be made at the Library for an additional $.25 per copy.

For New Or Renewal Passport for a Child Under the age of 16, all applicants must appear in person, and will need the following items:


  1. Child's Proof Of United States Citizenship Or Nationality
    These documents MUST BE mailed to the Passport Agency, photocopies are not accepted.

    • The Child's Certified Birth Certificate showing both parents names, or
    • Court Order of Guardianship
     
  3. Show Valid identification for BOTH parents
    • Drivers Licenses
    • U.S. Government Issued ID Cards

  4. Photographs - Two identical, 2"X2" square, full faced with plain white or off-white background. Passport photos are available at the Library for $10.00.

  5. Passport Fee - 2 checks or money orders only or 1 check/money order and 1 credit card (Visa/MC)

  6. If both parents cannot appear in person - The non-appearing parent may complete and notarize form DS-3053 or submit a notarized letter authorizing the other parent to apply for the child's passport. Click here for Form DS-3053

*  Renewal of adult passports less than 15 years old can be handled directly with the Department of State
